Notice of Availability of Form 1095-C

Form 1095-C (Employer-Provided Health Insurance Offer and Coverage) is available to employees upon request.

This form provides information about the health insurance coverage offered by the Library and may be helpful when preparing your federal income tax return. It is for your records only and does not need to be submitted with your tax return.

If you would like to request a copy of Form 1095-C, please email the Library’s Business Manager. Requests will be processed within 30 days.
